Step 1: Complete Your Resident Registration First
You are generally expected to register your address at Shinjuku City Office before enrolling in NHI. This can typically be done on the same visit.
Step 2: Visit the NHI Counter
Go to the Medical Insurance & Pension Division on the 4th floor of Shinjuku City Office (1-4-1 Kabukicho). The Foreign Residents Advisory Corner is on the same floor if you need language support.
Step 3: Submit Your Application
Fill out the NHI enrollment form and submit it with your Residence Card. If you have dependents, include their documents as well.
Step 4: Receive Your NHI Card
Your NHI card will be mailed to your registered address within approximately 2 weeks. Present this card at hospitals and clinics to receive the 70% coverage discount.
Step 5: Pay Your Premiums
Premium payment notices will arrive monthly. You can pay at convenience stores, banks, or set up automatic deduction from your bank account.
